When you submit our appointment request form, we collect: your name, phone number, email (optional), city, preferred appointment date/time, and details about your dental needs (e.g. number of missing teeth, budget range, treatment timeline). We also automatically record your IP address, browser/device information, referring page, and marketing-attribution data (such as UTM parameters, Google Click ID, and Meta/Facebook Click ID) when you visit or submit the form.
We use this information to contact you about your appointment, provide the dental services you've requested, and improve our marketing. We share a cryptographically hashed (not plain-text) version of your name, phone, and city with Meta (Facebook/Instagram) and Google for ad-measurement purposes (Conversions API and similar).
